Affordable services provided

Louisville Plastic Surgery Consultants work with a variety of local health organization insurance plans, Medicare and Medicaid. Not all procedures are covered by your health insurance. We request that payment of your consultation fees, co-payments and deductibles be paid in full during your initial consultation.

Please ask to speak with our patient coordinator to determine if your surgeon is a participating provider with your insurance plan before scheduling an appointment. Additional information on financial policies may be obtained by clicking on the link below or call our office at 502.899.9996.


Financial Policies

Louisville Plastic Surgery Consultants requests that payment of your consultation fees, co-payments and deductibles be paid in full during your initial consultation. Payment of elective surgical fees are due prior to surgery, due date will be determined at your initial consultation. Our patient financial consultant will provide you with a cost-analysis at your visit.

Louisville Plastic Surgery Consultants accepts payment of fees by: cash, American Express®, Visa, MasterCard® or cashier’s check. Check cards & all ATM debit transactions are not accepted as payment.

Louisville Plastic Surgery Consultants accepts cosmetic fee plans offered through Chase Health Advance, Surgery Loans and Care Credit. Please note these are outside services and not connected to our office.

Insurance

Louisville Plastic Surgery Consultant’s Surgeons accept most insurance plans, Medicare and Medicaid. Please ask to speak with our patient coordinator in the business office to determine if your surgeon is a participating provider with your insurance plan.

If your surgeon determines that your condition warrants surgical intervention, Louisville Plastic Surgery Consultants will work with your insurance plan to obtain precertification or prior approval before scheduling surgery. If your insurance carrier determines that “medical necessity” is not met, you will be responsible for the unpaid portion of your surgery. It is important to be aware that pre-certification and prior approval does not guarantee payment from your insurance plan.
